If you haven't uploaded or provided the following information, please be sure to bring these items along with you to your screening appointment in late spring/early summer:

-Child's birth certificate

-A state-issued photo ID

-A child's physical form within one year of your child's entry to Kindergarten (form below)

-Current immunization record

-2 forms of "Proof of Residency" (2 different recurring monthly utility bills, signed current lease, property tax forms, deed)

-IEP or 504 plan agreement (if applicable)

-Custody Agreement (if applicable)
